Blue Rivers Area Agency on Aging is accepting applications for a full-time Resource Developer and Service Coordinator. The hourly range is $22 to $24 (depending on experience), plus full benefits. Hours of work are generally Monday - Friday, 8:00 a.m. - 4:30 p.m.

Primary responsibility is to recruit and maintain service providers to work within the Aged and Disabled Medicaid Waiver Program and the Care Management Program. The main focus of these programs is to find service providers to provide the necessary care that individuals need to stay in their home safely. Recruitment is conducted by phone, face to face contact, advertisements, and/or letters. This position is also responsible for client screening with Medicaid waiver services/ assessments, development of Medicaid Waiver Plan of Services and Supports, and coordination of services for elderly and/or disabled individuals eligible for Medicaid that is assigned to you by your supervisor. Follow-up is required by regulation to assure quality of services provided, ensuring professionalism, and reliability for those served.

Travel will be necessary, but no overnight. Use of an agency or personal vehicle may be necessary with approval from your supervisor. Mileage will be reimbursed at the current Agency rate per the IRS.

Qualifications:

1. A current Nebraska License as a Registered Nurse, Bachelor’s or Master’s degree. Two years of Post High School education in Human Services or Equivalent Experience. Experience in Community Resource Development will be considered.

2. Two to Four years of equivalent Care Coordination/Case Management services.

3. Ability to work independently and communicate effectively in oral, written, and face to face contact.

4. Basic computer skills featuring Windows applications.

5. Knowledge of interviewing techniques is a plus.

6. Ability to communicate, organize, prioritize, time manage, and record keeping skills.

7. Must have the ability to relate to and work with older persons, co-workers, and the general public, must be reliable, and have a positive attitude.

Essential Responsibilities:

• Perform Senior Care Options Screens according to the regulation and protocol as determined by the Nebraska Department of Health and Human Services.

• Present choices of appropriate services to persons deemed eligible for Medicaid Waiver services through screening by Enrollment &Eligibility through the State of Nebraska and through community resources.

• Advocate for client and strive to maintain client safety.

• Identify the needs, available community resources, and matching of client and provider.

• Maintain documentation as required by Health and Human Services regarding each client, by hard copy and databases assigned to you.

• Advocate for providers, work closely with other service coordinators and your supervisor, and clients to resolve conflicts.

• Document all activities and efforts with appropriate parties and complete assessments per regulations.

• Provide all required documents to contracted providers to ensure eligibility.

• Maintain a professional relationship with existing providers in order to assure the quality and integrity of services the agency provides.

Other areas of responsibilities (to include but not limited to):

• Coordinate with various community agencies/organizations to inform them of the Aged and Disabled Medicaid Waiver Program and the Care Management Program to broaden the service delivery area.

• Ensure guidelines with the federal, state, and local professionals on program development, service development, and the development of a pool of service providers.

• Public speaking if needed to promote and give information concerning the Programs our agency provides.

• Responsible for preparing and submitting required reports, including data entry.

• Responsible for developing and maintaining knowledge in areas we serve through workshops and training.

Dusting, vacuuming, cleaning floors, taking out trash, clean the bathroom, clean the kitchen, laundry, make beds, wash dishes, shop for essential groceries, and light meal preparation.

Handyman Contractors provide yard work or snow removal.

*Clients are responsible for providing their own cleaning supplies and ice melt.

Rates begin at $14.00 for Housekeepers and $20.00 for Handyman Contractors. Work as many hours each week as you want, up to 40 hours. Flexible schedules - set your own work schedule.

Blue Rivers Public Transportation is accepting applications for an on-call Substitute Driver. Hourly wage is $14.00 an hour. Hours of work are generally Monday - Friday, 8:00 a.m. - 4:30 p.m.

A Substitute Driver is responsible for the safe transportation of all riders within the town of Beatrice and occasionally out of town.

Education, Certifications, and Licenses:
• High school diploma or equivalent required, or equivalent job experience preferred.

• Must have a valid state issued Driver’s license.

Functions:
• Demonstrate safe and confident driving skills in small and large cities.

• Deliver passengers to their desired destination in a timely and safe manner.

• Must be able to drive a larger than usual vehicle with a variety of blind spots safely.

• Return passengers in safe and courteous manner.

• Occasionally, deliver the monthly newsletter, and/or other materials as needed to Senior Centers on route.

• Attend all mandatory training and staff meetings.

• Subject to a pre-employment and a random drug/alcohol testing schedule conducted by a third party as well as a background checks of Social Services, Department of Motor Vehicles and the Nebraska State Patrol.

Physical Demands and Typical Working Conditions:
• Standing, bending, twisting, turning.

• Lifting and carrying up to a total weight of 30 lb. sacks into the vehicle and the passengers front door.

• Should be able to climb stairs if necessary.

Blue Rivers Public Transportation is accepting applications for a Part-Time Driver. Hourly wage is $14.50 an hour, plus benefits. The hours of work are Monday - Friday, 8:00 a.m. – 2:00 p.m., with a half hour lunch break. Position may become full time if demand and ridership increase.

A Part-Time Driver is responsible for the safe transportation of all riders within the town of Fairbury and occasionally out of town.

Functions:

• Demonstrate basic knowledge of computer programs such as Windows and the related programs within the Transit Industry (Zonar)

• Coordinate the demands of the passenger with the resource of the vehicle for transportation services.

• Must be able to drive a larger than usual vehicle with a variety of blind spots safely.

• Maintain courteous relations with all they may encounter daily,

• Demonstrate a thorough working knowledge of the area the driver is working within.

• Demonstrate the ability to use either a radio system or telephone to take or pick up passengers to the appropriate destinations

• Attend all mandatory training and staff meetings.

• Subject to a pre-employment and a random drug/alcohol testing schedule conducted by a third party.

• High school diploma or equivalent required, or equivalent job experience preferred.
